Severe nausea, vomiting, or dehydration
Stay well-hydrated to avoid complications, especially if experiencing gastrointestinal symptoms.

Kidney issues or kidney failure
Vomiting and diarrhea may worsen kidney problems in those with pre-existing conditions. Dehydration may require hemodialysis.

Signs of thyroid cancer (Thyroid C-Cell Tumor)
Look out for hoarseness, difficulty swallowing, neck swelling, or shortness of breath. (See Black Box Warning below.)

Pancreatitis
Severe abdominal or back pain that does not go away may indicate inflammation of the pancreas.

Gallbladder disease
Symptoms include upper abdominal pain, fever, yellowing of the eyes or skin, nausea/vomiting. Further evaluation may be required.

Hepatitis or liver issues
Includes jaundice or elevated liver enzymes.

Diabetic retinopathy
Vision changes in those with type 2 diabetes, especially those with prior eye complications.

Severe gastrointestinal disease
Avoid use if you have a history of serious gastrointestinal disorders.

Low blood sugar (hypoglycemia)
Rare with GLP-1s, but possible when combined with insulin or sulfonylureas. Symptoms: shakiness, dizziness, confusion, slurred speech, sweating, anxiety, or fainting.
Immediate Action: Consume 15–20g of sugar (e.g., juice, candy, soda).

Increased heart rate

Worsening depression, mood changes, or suicidal thoughts

Allergic reactions
Swelling of the face, lips, throat; rash, itching, trouble breathing, rapid heartbeat, or fainting.

Alcohol & Medication Interactions
Avoid alcohol. Compounded semaglutide may interact with both prescription and over-the-counter medications.

Gastrointestinal Effects on Oral Drugs
Delays gastric emptying and may affect absorption of oral medications.

Hormonal Medications
If taking thyroid or hormonal medications, consult your prescribing doctor to adjust timing.

Narrow Therapeutic Drugs (e.g., Warfarin)

🧬 BLACK BOX WARNING – Thyroid C-Cell Tumors
GLP-1 receptor agonists like semaglutide and tirzepatide are contraindicated in patients with:

Personal or family history of medullary thyroid carcinoma

Multiple endocrine neoplasia syndrome type 2 (MEN 2)

In animal studies, GLP-1 drugs have caused thyroid tumors. It is unknown if this risk applies to humans.
Contact your physician immediately if you experience:

Neck swelling or lumps

Hoarseness

Difficulty swallowing

Shortness of breath
